POSITION Treatment Foster Care Living Case Manager
POSITION SUMMARY The Case Manager will work with the Director in all aspects of a Treatment Foster Care program. This individual is responsible for overseeing the well-being of children who have been removed from their homes due to abuse, neglect, or other circumstances. The case manager will provide, coordinate and facilitate necessary services to foster youth and their foster caregivers to support the youth’s permanency goal.
SPECIFIC RESPONSIBILITIES include, but are not limited to: include, but are not limited to: include, but are not limited to:
Maintain necessary state relationships with licensing and other regulatory bodies, including familiarity with relevant rules and regulations.
Assist Director in recruiting prospective Treatment Foster parents and arrange training for those chosen for licensing.
Assist Director with on-going training for and evaluation of all program participants, including the meeting of training standards for licensing.
Coordinate periodic staffings whereby service providers, referring agents, and parents or significant others come together for treatment plan review and direction.
Keep appropriate records/statistics in order to facilitate and enhance program evaluation efforts.
Function on agency quality assurance committees or other special committees as assigned.
Attend regular supervisory conferences with planned agenda.
Enhance professional growth by reading, attendance at conferences, and in-service training.
Assume additional responsibilities as assigned by Director of Foster Care Services.
Coordinate the development, review, and implementation of services. Case management and behavior plans and treatments provided to foster youth.
Assists and advocates on behalf of foster youth to secure needed resources, services and supports.
Collaborates with foster parents and youth in development of crisis plans and provides crisis assistance and coaching as needed.
Assist youth with development of skills that will enhance and improve function in areas that include, but are not limited to interpersonal relationships, anger management, coping, and mental health needs.
Completion of diagnostic assessments, individual service plans and corresponding documentation as applicable and appropriate in a timely manner.
Work in conjunction with multidisciplinary team.
Conduct visits with children and foster care givers that include the ongoing assessment of child safety, permanency and well-being.
Perform case management responsibilities in accordance with state and federal laws, Oesterlen policies, procedures and council on accreditation standards.
QUALIFICATIONS
Bachelors Degree in Social Work or related field required; must be licensed or license eligible (LSW). Previous experience with children and youth in foster care preferred. Must have excellent planning and time management skills, be able to work with a wide variety of professionals and non-professionals.

Essential Functions of Work:

Documents service activity notes daily on clients’ activities, behavior patterns, and changes in behavior patterns, medications. Reviews and documents in shift report logs, shift logs, companion files, etc. Obtains required program information; inventory client’s valuables.
Administering Medications:
Takes vitals; counts control substance medications; supervises self-administered medications. Conducts security/safety checks; monitors visitors/clients for contraband such as knives, razors, belts; conducts searches; removes dangerous items; performs and/or assists with restraints. Utilizes infection control procedures when necessary. May shop for supplies/clothing for clients; assists with the preparation and dispensing of snacks and meals. Participates in meetings as required; converses with staff on the client’s progress or lack of progress. Performs general office duties; answers telephones; communicates with the public, police, DCFs, schools, hospitals, etc.; links clients to needed services; makes doctor appointments; files, types, copies; receives food shipments; order supplies, etc. Performs general cleaning responsibilities such as laundry, mopping, garbage removal, etc. Attends training /in-services as required. Controls verbally and/or physically abusive and aggressive clients and performs aggression control techniques; assists with restraints when necessary. Communicates effectively with suicidal, mentally, emotionally, and/or psychologically disturbed clients in person and over the telephone to diffuse potentially life-threatening situations. Transports clients to other agencies, hospitals, homes, community activities, AA meetings, etc.

POSITION: Service Coordinator
POSITION SUMMARY: Service Coordinator has responsibility for specific supportive activities to a given family as decided by the Service Coordination Team (Child/Family Team). Hours of service provision will vary and be mostly dependent upon the needs and schedule of the family.
SPECIFIC RESPONSIBILITIES include, but are not limited to:
1. Identify child and family needs by using the Family and Children First Counsil state approved assessment tool.
2. Facilitate service coordination services that are individualized and strength based, including case management, planning services, and provide direct services to families to implement planned interventions/services as identified.
3. The provision of direct services could include mentoring to child and/or parent; parenting skill building, household management skills; linkages to community-based services; transportation; and educational advocacy to meet the child’s educational goals.
4. Assist assigned families to become aware of and utilize formal and informal community support persons and activities that align with family strengths and goals.
5. Participate in regular family meetings (normally twice a week, but a minimum of once per week) to assess appropriateness of current involvement and be aware of the need for a shift of activity focus and/or degree of involvement.
6. Assist assigned families to achieve maximum independence through coordination of personal strengths and community resources.
7. Meet with the Service Coordination Program Coordinator for weekly supervision to review each family’s level of participation and progress.
8. Attend Service Coordination Team, which is a team built of family members, natural supports and informal supports, as well as community agencies. Keep the Service Coordination Team informed of progress on a weekly basis. If Service Coordination Provider is the Team Lead, then facilitation of the Service Coordination Team bi-weekly or monthly (at minimum) becomes necessary.
9. Maintain relevant documentation of service provision and involvement.
10. Attend designated orientation/training relative to the cultural needs, complex needs, and characteristics of families served, and various community resources and organizations that may assist families.
11. Assume additional responsibilities as assigned by Supervisor and/or Executive Director.

School Based Mental Health Counselor

POSITION SUMMARY
The school-based Mental Health Counselor will be responsible for providing services related to the delivery of mental health care to assigned clients at the designated school location and will provide consultation services to the school staff and administrators to support the overall mental health goals of the client and the school.
RESPONSIBILITIES
§ Provide direct mental health services to include intake assessment, individual/family/group therapy to assigned clients.
§ Meets with families and other interested parties in order to exchange necessary information during the assessment/treatment process.
§ Acts as a client advocate in order to coordinate required servicers or to resolve emergency problems in crisis situations.
§ Guides clients in the development of skills and strategies for dealing with identified problems.
